TranslateTeen Parent Connections
The Teen Parent Connections (Tcp)/Adolescent Family Life Program (Aflp) provides continuous case management to pregnant and/or parenting adolescents and their infants within a comprehensive local network of services and resources with the goals of reducing the incidence of poor pregnancy outcomes, subsequent pregnancies, and assisting them in improving the quality of their health, social, and economic well-being. Using a multi and trans disciplinary support network, Tcp/Aflp delivers long-term, comprehensive and future oriented home-based case management to high risk pregnant or parenting adolescents through the age of twenty.

Target Outcomes
Short Term Outcomes
- Client is physically active daily
- Client receives WIC
- Client attempts breastfeeding
- Child is current on immunizations
- Client attends school
- No tobacco and/or AOD use by client
- Client learns about accurate and consistent contraception use
- If sexually active, client always uses contraception
Long Term Outcomes
- Improved health and well being of pregnant and parenting teens and thereby their children over the life course
- Increased educational advancement among pregnant and parenting teens and thereby improved health through effects on income, health knowledge and behavior, socioeconomic status and support
