Upstream strives to support community-wide implementation of a new and smarter generation of prevention focused practices.
We recognize that the evidence informing Portfolio practices is ever developing. To ensure practices listed on the Portfolio represent current best practices, we ask organizations to renew their Portfolio membership every two years. To renew, membership organizations need to complete an outcomes report using the the template linked below. Alternately, organizations with completed evaluations may be eligible to submit an existing evaluation report that measures prioritized program outcomes.
