My Life
Youth transitioning from foster care to independence are most successful when they are surrounded by a team of adults and peers who are committed to helping them define their goals and make progress along their pathway to self-sufficiency in the areas of education, employment, housing, and wellness. We have found that when the youth's input is recognized as an asset in the planning process, the youth is much more connected and bought into the process. Additionally, youth are able to practice and exert their leadership skills in a safe environment focused on their own future.
The purpose of Voices Sonoma's My Life program is to provide emancipating foster youth a team of mentors who support young people's progress towards setting and meeting long-term goals.

Target Outcomes
Short Term Outcomes
- Youth will report that they feel supported by adult and peer mentors
- Youth can identify an adult to whom they can turn to for support
- Youth will have made progress towards at least one goa
- Youth will have easy access to well coordinated, appropriate and needed services
Long Term Outcomes
- Participants will complete high school
- Participants will enroll in post-secondary education/training
- Participants with have secure housing
- Participants will have self-sustaining income
